Rapid identification of extensive drug resistant (XDR) gram negative bacterial isolates in Payvand clinical and specialty laboratory, a six month survey

Background and Aim : Background The aim of this study was to evaluate the frequency of extensive drug resistant (MDR) gram negative bacterial isolates from different clinical samples in Payvand clinical and specialty laboratory. Methods : All gram negative bacteria isolated from different clinical samples from March to Sept ۲۰۲۰ were included. Blood agar, chocolate agar and MacConkey agar were used to access pure cultures .A bacterial suspension was prepared based on Vitec ۲ system for identification and Antimicrobial Susceptibility test (AST) using special cards. Statistical analysis was done by SPSS Version۲۱ software.Results : : ۴۱۲۵Urine, ۳۴ sputum samples and ۱ tracheal tube were included in this study. Of them ۴۸۶ Urine, ۳۲sputum and a tracheal tube was culture positive. ۳۱ XDR gram- bacteria were isolated from ۱۸ urine, ۱۲sputum and ۱ tracheal tube. The identified gram negative bacteria were: K. pneumonia (pneumonia (۲۰) and ozonai(۲)), E. coli (۳), Pseudomonas aeruginosa(۵) and Acinetobacter baumannii (۱). Also, ۶/۲۲ K. pneumonia (۲۷.۲۷%), ۱/۳ E. coli (۳۳.۳%), ۱/۵ P. aeruginosa (۲۰%) and ۱/۱ A. baumannii (۱۰۰%) were ESBL producers.Conclusion : In XDR cases the illness severity is higher than other situations and availability of effective antibiotics are more limited. So, rapid identification of XDR patients by using automated systems such as Vitec ۲ in less than ۷۲h can decrease the cost and mortality rate.
