چکیده مقاله:

A successful acute pain practice is often run in conjunction with a strong regional anesthesia service to providepatients with a multimodal approach to perioperative analgesia. Multimodal analgesia, in addition to oral andintravenous medications, may incorporate neuraxial and peripheral nerve catheters. These catheters have beenessential in decreasing opiate consumption, improving pain scores, and improving functional outcomes. Manyoptions are available for the treatment of acute postoperative pain ,including systemic ( i.e. opioid and nonopioid ) analgesics and regional (i.e. neuraxial and peripheral ) analgesic techniques. By considering patientpreferences and making an individualized assessment of the risk and benefits of each treatment modality , theclinician can optimize the postoperative analgesics regimen for each patient. Neural blockade with a localanesthetic may be used to identify specific pain pathways and to aid in diagnosis of the origin and site of thepain. Various acute pain syndromes respond to blockade of sympathetic or somatic neural pathways.Knowledge of the appropriate anatomic structures and careful technique are mandatory with this method.Therapeutic neural blockade with a local anesthetic plus a corticosteroid , an opioid, or rarely a neurolytic agentcan be useful in relieving a variety of acute pain syndromes. Neural blockade should not be viewed as stand –alone therapy for most acute pain syndrome ; rather it should be intelligently integrated into a comprehensivetreatment plan
