چکیده مقاله:

امروزه با وجود پیشرفت هایی در زمینه علوم دارویی و داروهای ضد قارچی ، عفونت های قارچی تهاجمی موجب مرگ و میر بسیاری از بیماران مبتلا به نقص سیستم ایمنی می شوند. قارچ های فرصت طلب کاندیدا و آسپرژیلوس، منجر به عفونت های پیشرفته و سیستمیک تهاجمی در این افراد م ی شوند. هرچند به طور قابل توجهی وضعیت سیستم ایمنی فرد تعیین کننده روند بیماری است، اما برای کنترل این عفونت ها استفاده از داروهای ضد قارچی مور لازم می باشد و درمان ضد قارچی زود هنگام برای نجات این بیماران ضروری می باشد. امروزه تعدادی عوامل ضد قارچی موثر با قابلیت تحمل پذیری بالاتر برای بیمار در دسترس است که در مقایسه با آمفوتریسین ب داکسی کولات بسیار گران قیمت تر می باشد . در درمان تجربی عفونت خونی کاندیدایی، فلوکونازول داروی انتخابی برای بیماران نوتروپنیک بدون سپسیس شدید یا شوک سپتیک بوده و م صرف قبلی داروهای آزولی م ی باشد. آمفوتریسین ب داکسی کولات یا کاسپوفانژین گزین ه ای برای درمان بیماران با مصرف قبلی دارو های آزولی می باشند. در بیماران نوتروپنیک درمان تجربی با آمفوتریسین ب داکسی کولات انتخاب اول در نظر گرفته شده است . برای درمان عفونت های ناشی از گون ه های کاندیدا آلبیکنس،کاندیدا تروپیکالیس و کاندیدا پاراپسیلوسیس، داروی فلوکونازول و برای درمان عفونت های ناشی از کاندیدا گلابراتا و یا کاندیدا کروزئی، داروی آمفوتریسین ب داکسی کولات یا کاسپوفانژین خط اول درمان توصیه می شود. دستورالعمل مهم درمان آسپرژیلوزیس تهاجمی به صورت درمان اولیه، درمان نجات دهنده و درمان ترکیبی در بیماران بد حال م ی باشد . وریکونازول،کاسپوفانژین (اگر به عنوان درمان اولیه استفاده نشود ) و آمفوتریسین ب لیپوزومی برای درمان بیماران مقاوم به درمان توصیه می شود. هم چنین درمان ترکیبی ب ا کاسپوفانژین همراه با وریکونازول و آمفوتریسین ب لیپوزومی باید در بیماران بد حال در نظر گرفته شود . در تصمیم گیری های بالینی برای تجویز یک دارو باید نتایج حاصل از مطالعات و راه کارهای منتشر یافته و هم چنین وضعیت صدور مجور و هزینه های برآوردی آن در نظر گرفته شود.
