Background: Nursing documentation has been recognized as a critical component of professional practice for nurses since the beginning of nursing as a profession .In nursing healthcare systems documentation is a powerful tool to ensure communication between health workers and continuity of care for better patient outcomes. In addition, the knowledge, performance, and attitude of nurses towards nursing documentation will affect the quality of patients’ care. This study aimed to determination nurses’ knowledge, attitude and performance regarding nursing documentation in Selected Al-Basra City Hospitals in ۲۰۲۲-۲۰۲۳ .Methods: A cross-sectional study was conducted through the present study. (۳۰۸) nurses was chosen from four teaching hospitals in the province of Basra in Iraq. A proportion of nurses from each hospital was selected for the study through multistage random sampling. Arabic version of questionnaires on "knowledge, attitudes, and Performance toward nursing documentation" are used for data collection. The validity of knowledge, attitudes, and Performance questionnaire was done by (۱۳) nursing experts .For the reliability of the questionnaire a pilot study was conducted, the result of pilot study showed that the questionnaire was reliable with internal reliability of correlated coefficient (r = ۰.۸۱). Descriptive and inferential statistics SPSS software version ۲۶ was used for data analysis, which was include.Results: The results of the study showed that (۷۵.۳%) of the participating nurses had average knowledge, (۹۶.۱%) their attitude was good and (۹۶.۸%) of the nurses had good performance towards nursing documentation. A significant relationship was found between the level of education (P= ۰.۰۰۰۱), the number of training (P= ۰.۰۰۰۱) and the knowledge of the nurses. Also a significant relationship was found between the name ward (P= ۰.۰۰۲), gender (P= ۰.۰۰۰۱), the number of nurses (p= ۰.۰۰۱), and the nurses' performance towards nursing documentation. Significant relationships were found between nurses' knowledge and attitude (P= ۰.۰۰۷), and between nurses' performance and attitude towards nursing documentation (P=۰.۰۰۵).Conclusions: Results of this study showed that performance and attitude of nursing documentation among nurses in Basra teaching hospitals were good. And the nurses' knowledge about nursing documentation was average. Sharing the research results to ministry of health and hospitals to distributing it with health directories in Iraq. In order to encourage nurses to attend to the training sessions to improve their knowledge and stay up to date on nursing documentation.
